Lid layout
The layout of the lid is taken primarily from the upper left hand corner of the Forget Me Not pattern. Start your stitching in the upper left hand corner, 2 inches down from the top and three inches over from the left hand side. Begin by stitching the complete motifs on pages 1, 2, 5, and 6. Add the complete motif from page 3 (shown here on the right hand side between the number 3 and letter S).
Now extend the motifs on the left side, charted in the pattern as half medallions. See the layout video tutorial for details on flipping a mirrored motif. You will work the entire motif for the smaller medallion from page 1. The larger motif on pages 1 and 5 will be extended 26 rows beyond the center line.

Motif A is found on pages 10, 11, and 15 and is worked almost in full. Do not work the last three rows on the right edge - I did and had to cut them off
Motif B is found on page 15 and is not worked in full. Work 29 rows from the top (missing 10 rows from the bottom of the motif)
Motif C is on page 10
Motif D is on page 9
Motif E is on page 15

Back layout
The back is comprised of three motifs, filled out with over 1 letters and numbers. I chose my initials and the year the box was completed, arranged in a seemingly random fashion so that it would follow the layout of the other sides of the box.
Motif A is found on pages 12 and 16 and is worked 15 rows past the center line. For details on flipping mirrored motifs, see the lid layout overview video.
Motif B is found on pages 6, 7, 10 and 11
Motif C is found as a half motif on page 16. Work the full motif by reversing the chart but not repeating the center line, as discussed in the lid layout overview tutorial.
Please note on the counts that those represent rows, or squares on a chart, ie they are two threads. So nine would be 9 rows or 18 threads between reference points.

Long side layout
The long sides have the most changes from the charted Forget Me Not pattern, but are laid out in a linear fashion that makes them easy to work. Start in the middle with motif A, at the bottom line, and then work outwards in either direction as outlined in the video overview. This will give you your bottom line.
From there, place motif I as it is charted on the Forget Me Not pattern in relation to motif B, working 4 rows above the center line. This will give you the top line of your design, and motifs can be worked outward in either direction along the top line once you have correctly placed motif I. For ease of counting, make sure I is your first motif worked after A-E (the bottom row motifs).
After the major motifs are in, fill in with the smaller numbered motifs, and the over 1 letters and numbers. As previously discussed, I chose letters and numbers that represented family members but these can easily be swapped out for letters that hold meaning for you - refer to the full chart of letters and numbers found in the Forget Me Not pattern.
Referencing the layout above, the order of work would be A (pages 4/8), B (pages 8/12), C (pages 1/2), D (pages 5/9), and E (pages 9/10) to establish the bottom line, then move up to motif I (pages 8/12), referencing its placement on the chart relative to motif B.
From there you can fill out the remaining motifs, found on the pages below:
F: page 4, worked 5 rows above the center motif line
G: page 15, worked 1 row above the center motif line
H: page 13, stopping 3 rows below the center motif line
I: pages 8/12. worked 4 rows above the center motif line
J: pages 3/4, worked 3 rows above the center motif line
Filler: 1 (page 6); 2 (page 12); 3 (found in the corner of the motif on page 1); 4 (page 15); 5 (page 9)
